Create a Smart Form
Steps:
Click "Settings" in Top Menu Bar
Click "Smart Forms" Tab
Click the “+ Add Smart Form” Button to open Create Smart Form Page
Enter Smart Form Name in Name Text Box
Select Offerings in Usable for Offering(s) Dropdown
Select Locations in Usable for Locations(s) Dropdown
Select Display Option in the Display Option Dropdown
(Optional: Make Smart Form Required) Click Smart Form is Required Check Box
(Optional: Enable Smart Form Review) Click Internal Review Required Check Box
Click “Add Question" Button
Enter Question in Question Text Box
Enter Description in Description Text Box
Select Question Type in Type Dropdown
Adjust Question Settings by clicking Settings Check Boxes
(If Needed: Multiple Choice and Dropdown) Enter Answer Option in Options Text Box
(If Needed: Multiple Choice and Dropdown) Click “+ New Option” Button and repeat Step 13
(If Needed) Repeat Steps 8-14 until all Questions are added to Smart Form
Click “Virtual Signature" Toggle to turn on/off Virtual Signature Requirement
(If Needed: Virtual Signature On) Add Instructions in the Instructions Text Box
(If Needed: Virtual Signature On) Add Description in Description Text Box
Click “Save Draft” Button
Publish a Smart Form
Steps:
Click "Settings" in Top Menu Bar
Click Smart Forms Tab
Find Unpublished Smart Form in Smart Form Table
Click on “More Options" Button
Click “Publish” Button
