Create a Smart Form

Steps:

  1. Click "Settings" in Top Menu Bar

  2. Click "Smart Forms" Tab

  3. Click the “+ Add Smart Form” Button to open Create Smart Form Page

  4. Enter Smart Form Name in Name Text Box

  5. Select Offerings in Usable for Offering(s) Dropdown

  6. Select Locations in Usable for Locations(s) Dropdown

  7. Select Display Option in the Display Option Dropdown

  8. (Optional: Make Smart Form Required) Click Smart Form is Required Check Box

  9. (Optional: Enable Smart Form Review) Click Internal Review Required Check Box

  10. Click “Add Question" Button

  11. Enter Question in Question Text Box

  12. Enter Description in Description Text Box

  13. Select Question Type in Type Dropdown

  14. Adjust Question Settings by clicking Settings Check Boxes

  15. (If Needed: Multiple Choice and Dropdown) Enter Answer Option in Options Text Box

  16. (If Needed: Multiple Choice and Dropdown) Click “+ New Option” Button and repeat Step 13

  17. (If Needed) Repeat Steps 8-14 until all Questions are added to Smart Form

  18. Click “Virtual Signature" Toggle to turn on/off Virtual Signature Requirement

    1. (If Needed: Virtual Signature On) Add Instructions in the Instructions Text Box

    2. (If Needed: Virtual Signature On) Add Description in Description Text Box

  19. Click “Save Draft” Button

Publish a Smart Form

Steps:

  1. Click "Settings" in Top Menu Bar

  2. Click Smart Forms Tab

  3. Find Unpublished Smart Form in Smart Form Table

  4. Click on “More Options" Button

  5. Click “Publish” Button
